A clear enquiry helps a professional reply usefully. Say where you found the insect, such as a mattress seam or a sofa. Mention whether you kept it, because an intact specimen is the strongest evidence.

If you rent your home, say so in the notes. The service menu has an option for rented homes, since your next steps may differ. Please do not add card numbers, passwords or health details. The form needs only your name, phone number and postcode, and everything else is optional.
